Part number 1274 6001. While this is all that is available, and looks quite good (see bottom photo), the switch could be better. Screws are a little long for most apps and may need to be cut. Connections need to be soldered QUICKLY with a HOT iron or surrounding plastic will melt. KNOW what you are doing before buying, please.
